Financial Assistance Guidelines

Scope – Within its financial resources the CMPNN will offer its community financial assistance for patients who need to travel to see an MPN specialist not located in their immediate vicinity.

Such assistance will be based on the following guidelines:

  • Financial assistance will be available to community individuals required to travel to and from a medical centre which is not in their immediate vicinity.
  • Financial assistance will cover Air/land travel expense, related accommodations expense, related food expense and any and all other specific expenses directly related to an MPN specialist appointment.
  • Financial assistance will be available upon request from individuals who are in financial need.
  • The applicant will not have to substantiate his/her financial position. Applicants will be considered based on the trust and honour system.
  • Financial assistance will be limited to a maximum of $500 per individual travel related expenses.
  • Applications are to be submitted for consideration in full confidentiality to the executive committee.
  • Applicant may approach any of the executive committee members either in writing or verbally.
  • The executive committee members are: The Chairperson, Vice Chair, Treasurer and Executive Director.
  • The executive committee will report to the board of Directors the amounts of funds allocated to financial assistance without disclosing any other details such as patient’s names, provinces or any other specific details regarding the fund’s allocation.
  • Financial assistance is to be offered prior to the related travel arrangements finalization within a reasonable timeframe as an advance covering upcoming estimated related expenses.
  • Once the travel related to this financial assistance is completed backup documentation supporting this assistance are to be provided. All such documentations are to be kept in full confidence by the Treasurer and is not to be disclosed unless otherwise legally required specifically by government law or court order.
  • Any funds not utilized or not supported by backup documentations are to be returned to CMPNN via the Treasurer.
